Primary Care Family Health Centers

Samaritan Medical Center is striving to meet the primary care needs of the community through a comprehensive network of community-based primary care centers. Currently, five Samaritan Family Health Centers exist throughout the community to meet the primary medical care needs of the region. These facilities are staffed by physicians, physician assistants and/or nurse practitioners, and provide a full continuum of care for an individual's lifetime, from birth through senior years.

In 2011, our four family health clinics at Clayton, Watertown, LeRay and Lacona were all recognized for achieving Patient Centered Medical Home Level 3 Designation by the National Committee for Quality Assurance. This is the highest level of accreditation.

Our largest Family Health Center is conveniently located in Watertown, New York. This clinic offers family practice, dermatology, plastic surgery, and infectious disease care.
