Department of Patient and Family Services at Samaritan
Medical Center is dedicated to assisting patients
and their families with post-hospital care planning.
We understand that a patient's recovery is closely
tied with their personal lives and social environment.
As patient advocates, we are committed to assuring
that you and your family experience full participation
in planning for post-hospital care. As you
make your plans, we provide emotional support, choice
of services, education, and knowledge of community,
financial, and social resources.
Specific
Activities
Arranging
services at home to assist in the patient's continued
recovery
Facilitating
transfer to other services or facilities if
the patient needs further treatment beyond acute
hospitalization before returning home
Assisting
patients and families with nursing home placement
Providing
financial education and guidance in the payment
for necessary post-hospital services
Offering
emotional support during a crisis event
Assisting
patients and families to cope with the impact
on family life when a serious illness or injury
occurs
Availability
Our
office is open from 7:30am to 6:00pm, Monday through
Friday. You may reach a social worker during
the week by calling 315.785.4150, or on weekends
by calling 315.785.4516. You are also free
to leave messages on our answering machine.
